We also provide Case Management Services to assist individuals with serious and persistent mental health disorders in gaining access to needed medical, social, educational and/or other services to help them improving their quality of life. Case Managers work with clients to ensure continuity of care through assessment, monitoring, individualized planning and linking to community resources.
Case management may also serve to provide necessary coordination of non-medical services such as vocational rehabilitation, education and employment, when the services provided enable the individual to function at the highest level.
Case Managers are responsible for educating, helping and linking consumers to the different community resources:
